Doch bei einem Teil der Patienten verliert das Medikament mit der Zeit seine Wirkung. Wir erkl\u00e4ren, warum das so ist und wie Medikamenten-Monitoring zu einem langfristigen Therapieerfolg beitragen kann.<\/strong><!--more--><\/p>\n<p>Ob bei Rheuma, Psoriasis oder bei chronisch entz\u00fcndlichen Darmerkrankungen: Das Medikament Adalimumab (ADM) hat die Therapieoptionen f\u00fcr viele Patienten deutlich verbessert. Als humaner monoklonaler Antik\u00f6rper gilt es als sehr effektiv und gut vertr\u00e4glich. Wie auch andere so genannte TNF\u03b1-Blocker bindet Adalimumab an den Tumornekrosefaktor-\u03b1 (TNF\u03b1) und hemmt dessen Wirkung. In der Folge sch\u00fcttet der K\u00f6rper weniger Entz\u00fcndungsbotenstoffe aus und die Entz\u00fcndung bessert sich. Doch bei einem Teil der Patienten wirkt Adalimumab nicht, oder es verliert mit der Zeit seine Wirkung. Mitunter hilft dann eine Dosiserh\u00f6hung. Ursache f\u00fcr den Wirkungsverlust kann aber auch sein, dass sich Antik\u00f6rper gegen das Medikament gebildet haben \u2013 so genannte Anti-Drug-Antik\u00f6rper (ADA). Diese Antik\u00f6rper neutralisieren die Wirkung von Adalimumab. In diesem Fall hilft nur ein Wechsel des Medikaments.<\/p>\n<p>Therapeutisches Medikamenten-Monitoring (TDM) kann den Arzt dabei unterst\u00fctzen, die richtige Therapieentscheidung zu treffen. Zwei Tests sind hier entscheidend:[\/vc_column_text][mk_icon_box icon=&#8220;&#8220; title=&#8220;Talspiegel-Messung&#8220; icon_color=&#8220;&#8220; icon_circle_color=&#8220;&#8220;]Um optimal wirken zu k\u00f6nnen, m\u00fcssen TNF\u03b1-Blocker wie Adalimumab in einer ausreichenden Konzentration im Blut vorliegen. Die Bioverf\u00fcgbarkeit kann sich jedoch von Person zu Person unterscheiden. Im Rahmen eines Medikamenten-Monitorings kann regelm\u00e4\u00dfig vor jeder neuen Infusion die Talspiegelkonzentration im Blut \u00fcberpr\u00fcft werden. Dies erm\u00f6glicht es dem Arzt, die Dosis individuell an den Patienten anzupassen, wodurch eine optimale Wirkung sichergestellt wird.[\/mk_icon_box][mk_icon_box icon=&#8220;&#8220; title=&#8220;Antik\u00f6rper-Messung&#8220; icon_color=&#8220;&#8220; icon_circle_color=&#8220;&#8220;]Wird bei der Talspiegel-Messung festgestellt, dass die Adalimumab-Konzentration unterhalb des therapeutisch erforderlichen Wertes liegt (&lt; 1 \u00b5g\/ml), sollte das Blut zus\u00e4tzlich auf Anti-ADM-Antik\u00f6rper getestet werden. Auf Basis der Antik\u00f6rper-Messung kann dann eine Therapieanpassung vorgenommen werden: Bei einer niedrigen Menge an Anti-ADM-Antik\u00f6rpern sollte die Dosis erh\u00f6ht bzw. das Intervall reduziert werden. Patienten mit einer hohen Menge an Anti-ADM-Antik\u00f6rpern sollten hingegen mit einem anderen Medikament behandelt werden.<\/p>\n<p>Die Talspiegel-Messung als auch die Antik\u00f6rper-Messung liefern dem Arzt wichtige Informationen, die f\u00fcr den weiteren Therapieerfolg entscheidend sind.
